A DOT physical exam is the evaluation required in order to operate a commercial vehicle. It is referred to as a “medical fitness for duty” examination, which ensures that each driver is capable and healthy enough to successfully complete the duties associated with commercial driving.
A DOT physical exam is valid for up to 24 months. The medical examiner may also issue a medical examiner’s certificate for less than 24 months when it is desirable to monitor a condition, such as high blood pressure.

Due to COVID-19 and the President’s declaration of a national emergency, the Federal Motor Carrier Safety Association (FMCSA) and many of the State Driver’s License Agencies have granted waivers from certain regulations applicable to commercial motor vehicle drivers. The regulations have resulted in an increased number of expired driver’s licenses among these commercial drivers. Unfortunately, many MEs are mistakenly refusing to provide MECs to the drivers.
Medical Examiners may conduct exams and issue MECs whether or not a driver has an expired driver’s license or not. The law states that:
Certified MEs listed on the National Registry are authorized to conduct examinations of, and issue MECs to, any driver that meets the physical qualification standards regardless of whether or not they have a current expired or unexpired driver’s license. The ME is only required to use the driver’s license to verify the identity of the person they are examining. An expired license is not a reason to refuse to conduct a physical qualification examination or to not issue the qualified driver an MEC.
